  • We have an exciting opportunity for a motivated Physical Therapist to live and work in Gunnison Valley. Gunnison Valley Health is accepting resumes for a PT to be employed by the hospital system but work in a multidisciplinary department which covers inpatient, some senior care, home health, swing bed and pediatric patients with an emphasis on outpatient excellence and growth. This position will have an emphasis on home health and senior care and is a great opportunity to integrate with quality rehabilitation staff possessing developed specialties in athletic medicine, pelvic health, post-concussion, oncology and occupational medicine. We also work closely with our Family Medicine Clinic and local orthopedic surgeons including a fellowship trained hand surgeon.

    POSITION PURPOSE

    Provision, direction, supervision, and evaluation of physical therapy services to patients in the home in accordance with agency policies and procedures, applicable health care standards, and governmental laws and regulations.

    POSITION QUALIFICATIONS

    Education: Graduate of a physical therapy curriculum approved by The American Physical Therapy Association, or The Committee on Allied Health Education and Accreditation of the American Medical Association, or The Council on Medical Education of the American Medical Association and the American Physical Therapy Association; or prior to January 1, 1966, was admitted to membership by the American Physical Therapy Association, or was admitted to registration by the American Registry of Physical Therapist, or has graduated from a physical therapy curriculum in a 4 year college or university approved by a State department of education; or has 2 years of appropriate experience as a physical therapist, and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service except that such determinations of proficiency do not apply with respect to persons initially licensed by a State or seeking qualification as a physical therapist after December 31, 1977; or was licensed or registered prior to January 1, 1966 and prior to January 1, 1970, had 15 years of fulltime experience in the treatment of illness or injury through the practice of physical therapy in which services were rendered under the order and direction of attending and referring doctors of medicine or osteopathy (Medicare Conditions of Participation for Home Health Agencies)

    Experience: At least one year appropriate clinical experience, preferably in home healthcare.

    Licenses/Certification: Licensed in the State of Colorado, valid drivers license, current automobile insurance, reliable transportation, and current CPR certification.

    ESSENTIAL FUNCTIONS

    • Projects a professional manner and image.
    • Prepares clinical and progress notes in accordance with agency policy.
    • Explain and operate equipment used as appropriate for procedures and treatment if necessary.
    • Involve client as able in decision making and planning of care.
    • Identify and plans for safety needs of the adult patient and initiates referral(s) as appropriate and necessary
    • Provide information on community support services and contact Clinical Supervisor to arrange referrals as appropriate.
    • Uses policy and procedure manuals, and other reference materials to ensure proper course of action.
    • Provides accurate and timely information using discretion and protects confidentiality of information.
    • Does not let personal conflicts interfere with work performance.
    • Participates in opportunities to improve practice as evidenced by involvement in data collection, task forces or projects.
    • Demonstrates prioritizing skills for delivering care.
    • Reports to work on time with no more than 1 tardy per pay period. Works as scheduled. No identified pattern of absence
    • Demonstrates ability to present ideas and information in a concise, effective and interpersonally appropriate manner through written and oral forms.
    • Demonstrates the ability to take responsibility and ownership for the outcome of all actions and decisions with particular emphasis on customer satisfaction and fiscal performance
    • Demonstrates the ability to support and implement effective change in order to enhance organizational performance.
    • Demonstrates clinical competency.
    • Utilizes time management strategies to complete patient care assignments within shift
    • Administers, interprets, and evaluates test and measurements of body function and structures in the application of therapeutic procedures.
    • Performs client evaluations using muscle, sensory, joint, gait, and functional availability test.
    • Confers with the physician in the determination of treatment modalities.
    • Participates in the development and periodic review of the Physician's Plan of Care (POC).
    • Instructs the client/family in therapeutic procedures, adaptive measures, equipment use, and preventative techniques.

    • Supervises and evaluates the performance of Physical Therapy Assistant and of Home Health Aides performing services as an extension of therapy care.
    • Provides documentation of additional supervision, instruction and evaluation of P.T.A.'s and Home Health Aides regarding their application of physical therapy services.
    • Utilizes special skills in treating clients to relieve pain, restore function to maximum capacity, compensate for lost function, establish maintenance programs and promote client safety.
    • Identifies individual learning needs and pursues opportunities for professional enrichment by attending continuing education programs. Attends practice updates.
    • Participates in drills and emergency response training
    • May be called in for GVH disaster response.
    • Other duties as assigned.
